          I want to get my treatment done in TMH, what is the procedure?


              Following is the Out Patient Procedure at Tata Memorial Hospital:
  1. While coming to the hospital, bring a responsible attendant with you. So, when in need they can take important decisions on your behalf.
  2. Bring all relevant documents like referral letter, slides, CT scan, other reports with you. Also, bring medicines and medical documents relating to your present and past medical conditions.
  3. If you are an outstation patient, check your pre-existing medical conditions like high blood pressure, diabetes etc are in control. This may avoid delay in treatment and also reduce your stay in Mumbai.



REGISTRATION PROCESS AT TMH 

Registration process is a one-time activity for a patient .All patients (private& general) need to be registered prior to consultation and requires patient to be at the site to complete the registration process which includes creation of smart card, DMG allocation and generating a patient file. 

Registration Hours:

Open from Monday to Friday from 0800 - 1400 hours, Saturday from 0800 - 1215 hours for both Private and General OPDs.

Channels for registration :

Web Registration:
·  Visit https://tmc.gov.in and click on ‘Online patient services’ to begin the registration process.
·  After completing online registration a provisional registration number is issued to the patient.
·  Patient then takes provisional registration number to the registration counters to proceed with creation of file and smart card.

Electronic Kiosk:
· Patients can login to the KIOSK Application complete the registration process.
· After completing online registration a provisional registration number is issued to the patient.
· Patient then takes provisional registration number to the registration counters to proceed with creation of file and smart card.

Manual Registration:
· If the patient hasn’t registered online then the patient is ask to fill a form and submit it to the registration counters which involves patients to wait in queue for the form and submission of the registration form.
· At the respective registration counter, registration details will be verified, patient will be photographed and a general consent by electronic fingerprinting will be taken.
·  A smart Card will be issued bearing patients name and case file number.


Fee Structure: 

   General Patients: 
                        General patient do not have to pay any registration fees; but it is advisable to have a minimum deposit of   RS 1,000 in Smart Card.
Private Patients: 
                 The mandatory Smart Card deposit for private Indian national is RS 10,000 and RS 50,000 for foreign nationals.

Smart Card:
                 The Smart Card facility makes in-house transaction cashless and enables viewing of   medical records and avail of various travel concessions. For the ease of patients within the institution, electronic kiosks have been made available in convenient locations. SMS messaging services have been introduced for appointment reminders and the availability of their finalized reports.


New Patient Registration Process (General -Golden Jubilee Building):

·                           ü Register at the registration office located on Ground Floor of Golden Jubilee Building counter no. 55.
ü Patients fill the form and queue up outside Medical officers room GJB-G-53.Medical officer does the initial checks (views the reports) and allocates the DMG to patient.
ü  Patient then moves to the registration counter GJB-G-55 the counter staff scans the patient documents and generates a case file. Patient is asked to wait at waiting area for smart card generation.
ü  After the name been called out patient moves to sub counter where the counter staff takes a photograph of the patient, completes biometric scanning and generates a smartcard.
ü  Patient is then asked to deposit cash in the smart card.
ü  Patient moves to the cash counter at GJB first floor counter number GJB-111.
ü  After depositing cash case file is handed to the patient.
ü  Patient then moves to the respective DMG/OPD.

 Note : Incase if you are facing difficulty in the process, you can take the help of Public Relations Office/ Medical Social Worker/ Voluntary Social Worker.

New Patient Registration Process (Private-Homi Bhabha Building):

ü  Register at the registration counter located on the 1st floor, Homi Bhabha Building.
ü  Registration office will issue a case file and registration card with case file number on it. Keep the card safe. As this will enable speedy retrieval of the medical case each time you visit the hospital for treatment or consultation. This case file number is the unique identifier of your medical records.
ü You can choose your doctor. Otherwise, the authorities will assign you a doctor appropriate for your case.
ü If you are referred by your employer/insurer with prior credit arrangement with Tata Memorial Hospital, please contact their Accounts Department for extension of credit facility.


Registration Process for Foreign Patient
ü Foreign nationals must email their medical reports to internationalpatients@tmc.gov.in.
ü The doctor will review the records. An email will be sent to you with letter of appointment for your medical visa.
ü You must possess a valid medical visa at the time of registration. Attendant should also carry Medical X Visa.
ü You must deposit a sum of Rs 2 Lakh in Indian currency before undergoing specialized treatment including operation.
ü You need to deposit the Rs.50,000 at the time of registration .
ü   You must contact the Chief security Officer with an original passport and valid medical visa whenever admitted to the institution.
ü All the documents will be attested by the Security and Public Relations Department.

Appointment and Travel Concession


   You as a General or Private patient can choose to avail for travel concessions for road, rail and air    transport to your place of permanent residence. Therefore, ensure that you enter your nearest airport/railway station/bus depot correctly in the registration form to avail for travel concession.
Also, for your next visit, the doctor’s secretary or the registration office will issue an appointment slip. That slip contains the date, time of the next appointment.
You may also choose to avail travel concession for Road/Rail/Air transport to your place of permanent residence as per rules laid down by the concerned travel authorities.

Diagnostic Services


ü  After getting advice from your treating doctor, you will have to go to the diagnostics areas. There you have to give blood/body fluid samples. You may have to undergo diagnostic scopies/biopsies.
ü  If you are an outpatient, you will have to pay for all the services you avail. The secretary of the doctor will prepare a consolidated memo. Also, you will have to pay this amount at the cash counter located on the ground floor. Otherwise, if you are a credit patient, the value of the services availed will be debited from your account.
ü  The doctor will evaluate the report and will discuss the treatment options with you. Also, you can clear all the issues concerning your treatment.
ü  The doctor may ask you to take the treatment as an inpatient. Also, they will give you the cost estimate of the treatment. You may note that the estimated cost may escalate in case of any complications.
ü  After your evaluation, you will have to deposit the case file to the concerned secretary in the private OPD and Registration Office in the general OPD.
ü  You may take photocopy of the contents for further reference and records. Also, get the photocopies attested by the Public Relations Officer.


  How to avail for Rajiv Gandhi Jeevandayee Yojana Scheme?


Patient should be carrying yellow or orange ration card of Maharashtra state with annual income of less than Rs 1,00,000 per annum. They can avail for Rs 1,50,000 per year in treatment concession for your family. Patient will then be registered under BP category.

Is there a good but cheap hotel or accommodation nearby Tata Memorial Hospital in Mumbai for a patient getting Cancer treatment there.

If you are an adult, the medical social workers’ department also arranges free or subsidized accommodation at Borges Memorial Home in Bandra and other sites. Accommodation is available for both general and private category patients on the basis of first come first serve basis. You can stay there for 4 to 6 weeks, and is applicable for those in close regular follow-ups. You need to present your hospital smart card and discharge card to allow admission.



                         Diseases Management Group (DMGs or OPDs)


There are 11 Disease Management Groups (DMGs) formed for region specific cancer sites that provide evidence based diagnosis and, decide holistically on the treatment modality viz., surgery, chemotherapy, radiation and palliation as a combination or independent, for every individual patient. This ensured better outcome and quality of life for the cancer patient.
All the 11 DMGs operate from Homi Bhabha building, Golden jubilee building and Main building. Golden Jubilee building is basically for General category and Homi Bhabha building is for private category. Main building comprises of 2 common OPDs between general and private categories naming Pediatric Solid Tumors & Pediatric Hematolymphoid and 2 separate private OPDs.

Overview:

 

The DMG endeavor’s to deliver comprehensive patient care in an effective and efficient manner, besides conducting research that is directed towards optimizing treatment for malignancies related to bone & soft tissue.
To streamline its functioning, each subspeciality ensures that all patients attending the clinics, are seen on all days of the week.The preliminary evaluation is completed the first day of the visit itself. This ensures early initialization of the treatment process. Three types of joint clinics (JC) are conducted viz., mutli-disciplinary JC to plan treatment; clinico-pathological todiscuss diagnosis and staging process for challenging cases and the third is conducted to facilitate rehabilitation of patients with for musculoskeletal malignancies.

Overview:




The DMG – Gastrointestinal (GI) delivers comprehensive care to patients with gastrointestinal cancers as a multidisciplinary disease management group (DMG), rather than individual specialties. GI disease management group consists of surgical oncologists, medical oncologists, gastroenterologists, radiation oncologists, diagnostic and conventional and interventional radiologists, pathologists, nuclear medicine consultants, basic scientists, epidemiologist and clinical nutritionists other support services. Such as nutrition clinic, stoma clinic, familial cancer and genetics clinic, catheter clinic and psychiatric services are availed of when required. The group members are geared towards improving the speed and quality of service, alongside education and research. A medical social worker support group monthly meet for re-addressal of patients’ socioeconomic and logistic issues has been established.

Neuro-Oncology – DMG

Neuro-oncology DMG provides comprehensive care for patients with brain and spine tumors. This includes outpatient as well as inpatient services (both elective and emergency care). The entire clinical team along with the DMG coordinators and other trial staff ensures smooth coordination between the different clinical specialties and supporting departments. Paediatric Hemato-Lymphoid - DMG


This DMG provided comprehensive medical diagnostic, counseling management, follow-up and palliative services to children below 15 years of age suffering from Hematolymphoid malignancies. Majority families, of the patients with malignancies of highly curative nature, have severe socioeconomic constraints. The group recognized that intervention in these issues would help to save lives, and improve outcomes quantitatively and qualitatively. In order to achieve this, the group developed an extensive support system to provide financial aid, accommodation, nutrition, continued education of children, sporting and fun activities, outings and cultural programs as part of its service program.

Paediatric Solid Tumor - DMG


On the initial visit of the patient to the outpatient clinic, a detailed socioeconomic assessment and counseling is carried out to identify those requiring emergency management and those with severe financial constraints. The DMG has a weekly comprehensive social support system that ensures issues related to compliance to treatment and its abandonment; thereby increasing survival rates. Deserving patients are selected by the dietician, who provides free mid-day meals during their visits or stay in the hospital. DMG members consisting of various expertise meet regularly to discuss patient care, and administrative matters. Social Support Group meetings are attended by medical social workers and volunteers.

Uro - Oncology – DMG

The Urology Oncology DMG is a multidisciplinary group with aims to deliver optimum patient care as well as bring all clinical and translational research under one umbrella to ensure optimum outcomes.New registrations are evaluated jointly by all specialists on a daily basis. After a detailed evaluation and an appropriate workup is complete, patients are reviewed by senior DMG member (consultant) followed by discussion in Joint Clinic for a management plan. Uro-oncology DMG has Outpatient clinics on Tuesday and Thursday whereas the new files including emergency cases are seen daily.
